Essential Duties and Responsibilities
• Review and verify prescription orders for accuracy, appropriateness, and safety, including dose, indication, drug interactions, and contraindications.
• Evaluate patient-specific clinical information (e.g., diagnosis, lab values, medication history) to ensure optimal therapy selection.
• Make evidence-based recommendations to providers regarding therapy modifications, dosing adjustments, or alternative treatments when appropriate.
• Ensure compliance with applicable regulations, clinical guidelines, and best practices in oncology and specialty pharmacy.
• Coordinates with physician staff and service providers in planning and implementing patient care plans.
• Provide timely responses to provider inquiries related to medication therapy, dosing, side effects, and drug interactions.
• Respond to patient questions regarding medications, including administration, side effects, adherence, and general drug information.
• Identify and escalate potential medication safety concerns or adverse drug reactions.
• Document all clinical interventions, recommendations, and communications in the appropriate systems.
• Support prior authorization and medication access processes through clinical review when needed.
• Maintain up-to-date knowledge of oncology therapies, drug approvals, and treatment guidelines.
Accreditation and Compliance
• Adhere to all applicable pharmacy laws, regulations, and organizational policies.
• Maintain accurate and complete documentation to support compliance and audit readiness.
• Participate in quality assurance and continuous improvement activities as required.
Minimum Qualifications
• Doctor of Pharmacy (PharmD) or Bachelor’s degree in Pharmacy from an accredited institution.
• Active, unrestricted pharmacist license in the state of Washington.
• Minimum of 3 years of dispensing pharmacist experience; oncology or specialty pharmacy experience strongly preferred.
• Strong clinical knowledge of oncology medications and treatment protocols.
• Excellent communication skills for remote patient and provider interactions.
Work Environment
• Fully remote position.
• Requires prolonged computer use and frequent communication via phone and electronic systems.
Reporting Responsibilities
• Reports to Pharmacy Manager, Clinical Director, or designated supervisor.
• No direct supervisory responsibilities.
